Now Mark Sheets of your Graduate School are very much required.
Make photo-copies of your Transcripts and ALL mark sheets and get them certified by your high school Principal or Dean or Associate Dean or University. Any of them will work good (unless they are very specific)
Each copy of mark sheet has to be signed and Attested with the stamp of Principal or Dean or Associate Dean or University. Put all the attested mark sheets in ONE envelope . This envelope also has to be signed and attested outside after being glued.
You can also put Internship Completion Certificate along, again duly attested and signed as seen below.

1) Transcripts also include number of hours you have attended the classes in BDS from anatomy to oral surgery everything. Not every school gives it. But if you can get it , it will be great.
2) transcript may include a concise one page document with all your marks mentioned for every exam you took. This transcript is separate from the individual year mark sheets that you get.
3) There is also a single page document which also talks about your residency/Internship dates, your extensions and finally its completion in all the departments.
So, you have to send all individual year mark sheets and Transcripts #1,2,3 Degree certificate or even provisional if your school provides them!!
